YEAR 10 STUDENTS: IMMUNISATIONS - WEDNESDAY 16 AUGUST
The Department of Health provides a free immunisation program for Year 10 students. The School Based Immunisation Program (SBIP) offers the Meningococcal ACWY (Men ACWY) vaccine. All students require parent/guardian consent before they receive any vaccinations at school. This year the SBIP is transitioning to an online platform.

- Please use the VaccinateWA online platform to complete your child’s consent.
If you're not able to access the online system, you can request a hard copy form from school or print a copy of the School-based-immunisation-program-form.pdf (healthywa.wa.gov.au) Please sign and return form to the school even if consent is declined. NOTE: Paper-based consent forms will be entered onto VaccinateWA and you will receive an email confirming this.
For privacy reasons, anyone aged 14 or older can get their own immunisation history statements. They can do this:
- online using their Medicare online accountthrough myGov or the Medicare mobile app
- by asking their vaccination provider to print it
- by calling the Australian Immunisation Register
Year 10 Immunisations are scheduled for Wednesday, 16 August. Students absent on immunisation days will be offered a catch-up at the next scheduled SBIP visit.
Parents/guardians also have the option of accessing these vaccines through their local Service Provider (GP, pharmacy, or Community Immunisation Clinic). Note some GPs and pharmacies may charge a consultation fee.
